• Contact Information including First and Last Name provided at the time of purchase  of your Service Plan (usually indicated on your invoice)
• Date of Purchase
• Name of Store or Retailer where you purchased your Service Plan
• Plan Registration Number (if available and/or given to you)
• Furniture Items Purchased
